新疆地区汉族人上颌第一前磨牙根管系统解剖形态分析Morphological Analysis of maxillary first premolar in Xinjiang han population

摘要(Abstract)：
目的探讨新疆地区汉族人双侧上颌第一前磨牙根管系统解剖形态,为临床根管治疗提供影像学依据。方法收集100例双侧上颌第一前磨牙的口腔锥形束CT(CBCT)扫描图像,分析其牙根数目、根管数目及形态、两根管口间的距离、多根牙根分叉和多根管分叉位置。结果上颌第一前磨牙单根发生率为37.5%,双根发生率为62.5%;双根管发现率为94.0%,其中Ⅳ型为66.5%,Ⅱ型为23.5%,Ⅲ型为2.0%,Ⅴ型为1.5%,Ⅵ型为0.5%,单根管即Ⅰ型发现率为5%,三根管(2-3)型发现率为1%;两根管口间距离集中在2~4 mm,其中距离1~2mm有19颗牙(10.38%),~3 mm有96颗牙(52.46%),~4 mm有64颗牙(34.97%),~5 mm有4颗牙(2.19%)。上颌第一前磨牙多根牙根分叉以根中1/3处多见,而根管分叉位置多以根冠1/3处多见。上颌第一前磨牙单双牙根的发生率与性别有相关性(P=0.049),男性双根发生率多于女性,而左、右两侧上颌第一前磨牙单双牙根发生率差异无统计学意义(P>0.05)。结论上颌第一前磨牙根管系统解剖形态复杂,CBCT可为根管系统提供准确的依据。
