超声引导下原发性肝癌射频消融术后复发相关因素

目的分析原发性肝癌超声引导下射频消融术后复发的相关危险因素。方法选取2016年1月—2019年12月100例在新疆医科大学第一附属医院行射频消融术治疗的肝癌患者,随访3年观察入选患者复发情况,并根据患者有无复发分为复发组与对照组,采用单因素分析法和多因素分析法分析造成患者术后复发的相关因素。结果超声随访观察3年,共有16例患者复发,总复发率为16.0%。单因素分析结果显示:肝功能分级、病灶数目、病灶大小、消融范围等因素是造成患者术后复发的相关因素(P<0.05)。多因素分析结果显示,病灶数目、病灶大小、消融范围是患者术后复发的独立危险因素(P<0.05)。结论超声引导下射频消融术治疗原发性肝癌具有较好的远期疗效。而术后复发与肝功能分级、病灶数目、病灶大小、消融范围有密切相关性。

Abstract:

Objective: To analyze the related risk factors of recurrence after ultrasound-guided radiofrequency ablation of primary liver cancer. Methods: From January 2016 to December 2019, 100 patients with liver cancer treated by radiofrequency ablation were selected, who followed up for 3 years to observe their recurrence. The patients were divided into recurrence group and control group. Single factor analysis and multifactor analysis were used to analyze the related factors of postoperative recurrence. Results: A total of 16 cases were found to recur after 3 years of ultrasound follow-up, with the total recurrence rate was 16.0%. Through univariate analysis, liver function grade, number of lesions, size of lesions, ablation range and other factors are related to postoperative recurrence(P<0.05). Through a multifactorial analysis, lesion number, lesion size, ablation range were independent risk factors for postoperative recurrence(P<0.05). Conclusion: Ultrasound-guided radiofrequency ablation is effective in the treatment of liver cancer. However, postoperative recurrence was closely related to liver function grade, number of lesions, size of lesions and ablation range.
