目的研究中药疏肝养血联合心理疏导对血虚肝郁型产后抑郁症患者心理状态、血清神经递质和性激素水平的影响。方法以2018年1月～2019年1月唐山市4家医院妇产科收治的102例血虚肝郁型产后抑郁症患者为研究对象,按照随机数字表法分为2组,对照组给予心理疏导治疗,观察组在心理疏导治疗基础上给予中药疏肝养血汤治疗,连续治4周,比较2组患者心理状态[爱丁堡产后抑郁量表(EPDS)、焦虑自评量表(SAS)]、血清神经递质水平[5-羟色胺(5-HT)、多巴胺(DA)、去甲肾上腺素(NE)]、血清性激素水平[雌二醇(E2)、催乳激素(PRL)、孕酮(P)]和临床疗效的改善情况。结果治疗后,与对照组比较,观察组血清5-HT、DA、NE、E2水平升高,EPDS评分、SAS评分、PRL和P水平降低,差异有统计学意义(P<0.05);治疗后,观察组治疗总有高于对照组,差异有统计学意义(P<0.05)。结论中药疏肝养血联合心理疏导能够调节血虚肝郁型产后抑郁症患者血清神经递质和性激素水平,缓解抑郁程度,增强临床疗效。

Objective To observe the effect of Shugan Yangxue combined with psychological dredging on the psychological state, serum neurotransmitter and sex hormone level of postpartum depression patients with Xuexu Ganyu type.Methods A total of 102 cases of postpartum depression with Xuexu Ganyu type were randomly divided into two groups. The control group was treated with psychological counseling, while the observation group was treated with Shugan Yangxue decoction on the basis of psychological counseling for 4 weeks. The improvement of psychological state(EPDS and SAS), serum neurotransmitter level(5-HT, DA and NE), and the serum sex hormone levels(E2, PRL and P) of the two groups were compared, and the clinical effects of the two groups were evaluated. Results After the treatment, the scores of EPDS and SAS in the two groups were significantly reduced(P < 0.05), which in the observation group was significantly lower than that in the control group(P < 0.05); the serum levels of 5-HT, Da, and NE in the two groups were significantly increased(P < 0.05), which in the observation group were significantly higher than those in the control group(P<0.05); the levels of E2 in the two groups were significantly increased(P < 0.05), which in the observation group were significantly higher than those in the control group(P < 0.05); the levels of PRL and P in the two groups were significantly decreased, which were significantly lower than those in the control group(P < 0.05);the total effective rate of the observation group was 94.1%, which was significantly higher than that of the control group(78.4%)(P < 0.05). Conclusion Shugan Yangxue combined with psychological dredging can regulate the level of serum neurotransmitters and sex hormones in postpartum depression patients with Xuexu Ganyu type, which can alleviate the degree of depression and enhance the clinical effects.
