目的探讨慢性鼻-鼻窦炎(CRS)伴或不伴鼻息肉患者鼻内镜手术治疗后的生活质量。方法选取2010年1月-2012年12月于新疆医科大学第一附属医院进行鼻内镜下治疗伴鼻息肉的CRS患者60例(观察组),选取同期不伴鼻息肉的CRS患者60例作为对照组,两组患者均行鼻内镜下手术治疗。评估指标包括视觉模拟量表评分(VAS)、鼻腔鼻窦结局测试-20(SNOT-20)。评估术前与手术第3、6个月后患者的生活质量,并对两组评估结果进行比较。结果观察组60例患者中57例完成随访,失访3例,对照组60例患者中55例完成随访,失访5例。观察组与对照组患者鼻内镜术后第3、6个月总体VAS评分较术前明显改善(P均<0.05),但两组VAS评分差异无统计学意义(P>0.05);观察组与对照组术后第3、6个月SNOT-20评分较术前明显下降(P均<0.05),两组SNOT-20评分相比差异无统计学意义(P>0.05)。结论鼻内镜鼻窦手术用于治疗伴或不伴鼻息肉CRS疗效确切,能有效改善患者鼻部症状,明显提高生活质量。

Objective To investigate the quality of life of patients with or without polyps of chronic rhinosinusitis after endoscopic sinus surgery. Methods Methods From January 2010 to December 2012,60 patients with chronic rhinosinusitis and nasal polyps in the first affiliated hospital of Xinjiang Medica university were chosen as the study group. Meanwhile,the other 60 patients without nasal polyps of chronic rhinosinusitis were also chosen as the control group. Both the two groups' patients were used endoscopic sinus surgery. Visual analogue scale( VAS),the sino-nasal outcome test-20( SNOT-20) were included as the assessment indicators. The quality of life of patient in his pre-operation and 3- 6 months after operation was assessed,and the clinical efficacy of the two groups were compared. Results fifty-seven patients completed 6 months follow-up and three patients withdrew due to different reason in the observation group,in the control group five patients withdrew and fifty-five patients completed followup. VAS overall symptom score were significant differences in the observation group and the control group pre-operation and after the operation( all P < 0. 05),there were no significant statistically differences found between the two groups( P > 0. 05). SNOT-20 score got better markedly either( all P < 0. 05),while among them the differences were not significant statistically( P > 0. 05). Conclusion Endoscopic sinus surgery has an exact effect on the treatment of with or without nasal polyps of chronic rhinosinusitis,it can effectively improve the outcomes of patients including symptom and health-related quality of life.
