目的探讨快速康复护理对全膝关节置换病人术后膝关节功能恢复的效果。方法 2018年12月-2019年4月选取新疆某三甲医院72例全膝关节置换术(TKA)患者,随机分为对照组和干预组,各36例,对照组给予常规护理,干预组给予快速康复护理。比较两组患者疼痛评分、美国特种外科医院膝关节(HSS)评分、关节活动度(ROM),术后首次下床时间、直腿抬高时间、导尿管留置时间、住院时间、并发症的差异。结果干预组在术后2 h、6 h、1 d、2 d静息状态疼痛评分明显低于对照组(P<0.05);干预组患者出院时及术后1个月膝关节HSS评分均明显高于对照组,同时术后3 d、出院时及术后1个月ROM均大于对照组,差异具有统计学意义(P<0.05);干预组首次下床时间、直腿抬高时间、导尿管留置时间、住院时间、并发症的发生率明显低于对照组(P<0.05)。结论快速康复护理模式可以促进患者术后早期康复、缩短住院时间、提高患者术后膝关节功能恢复效果,具有临床推广价值。

Objective To explore the effect of rapid rehabilitation nursing on knee function recovery after total knee arthroplasty. Methods 72 total knee arthroplasty(TKA) patients were randomly divided into control group and intervention group, 36 cases in each group. The control group received traditional nursing, while the intervention group received fast rehabilitation nursing. Pain score, HSS score, ROM score, first time out of bed, straight leg elevation time, the indwelling of catheter, the length of hospitalization, and complications were compared between the two groups. Results The pain score of intervention group was lower than that of control group at 2 h, 6 h, 1 d and 2 d after operation(P<0.05); HSS score and joint activity were significantly higher than that of control group(P<0.05) at discharge and 1 month after surgery; ROM was larger than the control group at 3 days after surgery, discharge and 1 month after surgery, the difference was statistically significant. The first time out of bed, straight leg elevation time, indwelling time of catheter, hospitalization time and the incidence of complications of intervention group were significantly lower than those of control group(P<0.05). Conclusion Rapid rehabilitation nursing model can promote early rehabilitation, shorten hospitalization time and improve the effect of knee joint function recovery after operation. It has clinical popularization value.
