目的探讨中医外治创新行气通腹饼灸辅助治疗粘连性肠梗阻的临床疗效。方法选择2017年12月-2019年8月陕西中医药大学附属医院门诊、急诊诊断的66例粘连性肠梗阻(既往有手术病史,且排除消化道肿瘤)患者为研究对象,采用数字表法随机分为对照组33例,研究组33例。对照组给予常规治疗方法,包括上注下灌中药汤剂、胃肠减压、禁食水、补液、抗炎及维持水、电解质平衡等;研究组在此基础上给予中医外治创新行气通腹饼灸治疗。治疗结束后分析比两组患者相关指标的差异。结果与对照组比较,研究组患者首次排便排气时间、腹痛缓解时间、拔管时间、平均住院日、腹部平片气液平面消失时间均缩短(P<0.01);与对照组比较,研究组患者干预后第7天,SAS、SDS评分显著减少(P<0.05);与对照组比较,研究组患者的出院时躯体功能及角色、肢体疼痛、总的健康状况、活力、社会功能、情绪角色及心理卫生评分均显著提高(P<0.01);与对照组比较,研究组患者的临床疗效总有效率显著提高(P<0.05)。结论行气通腹饼灸辅助治疗操作简便,疗效可佳,适用于粘连性肠梗阻的治疗。

Objective To explore the clinical effect of Qi Tong abdominal cake moxibustion in treatment of adhesive intestinal obstruction. Methods A total of 66 confirmed cases with adhesive intestinal obstruction from December 2017 to August 2019 in outpatient and Emergency Room of the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine were selected as the subjects. These selected patients had a history of surgery and never been diagnosed with digestive tract tumors. By adopting digital table method, these 66 cases were randomly divided into control group(33 cases) and study group(33 cases). Patients in the control group were given routine treatment, including traditional Chinese medicine decoction, gastrointestinal decompression, fasting water, fluid rehydration, anti-inflammatory treatment as well as water and electrolyte balance. And the patients in the study group were treated with Qi Tong abdominal cake moxibustion on the basis of above mentioned routine treatment. After the treatment, the differences of the related indicators between the two groups were compared. Results Compared with the control group, time for the first defecation, abdominal pain relief, extubation, average hospitalization duration and gas-liquid plane disappearance were all shortened in the study group(P<0.01), SAS and SDS score declined significantly on the 7 th day after the intervention(P<0.05). Compared with the control group, the physical function, limb pain, general health status, vitality, social function, emotional role and mental health scores of the patients in the study group were significantly improved(P<0.01); Compared with the control group, the total effective rate of the patients in the study group was significantly improved(P<0.05). Conclusion Qi Tong abdominal cake moxibustion is easy and convenient to operate, which is effective in treating adhesive intestinal obstruction.
