目的 探讨α1抗胰蛋白酶(AAT)、血管内皮生长因子(VEGF)、白细胞介素-1β(IL-1β)与慢性阻塞性肺疾病(COPD)肺功能的关联性及检测价值。方法 回顾性选取2019年11月-2022年11月安徽省芜湖市第一人民医院呼吸内科收治的178例COPD患者的临床资料为研究组,另选取178例同期健康体检者的临床资料为对照组。所有研究对象均在晨起时抽取5 mL空腹静脉血,采取免疫比浊法测定AAT水平,采取酶联免疫吸附法测定血清VEGF、IL-1β水平,并测定肺功能[最大呼气中期流量(MMEF)、第1 s用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC]。统计两组血清AAT、VEGF、IL-1β及肺功能指标水平,分析AAT、VEGF、IL-1β与肺功能指标的关联性。结果 与对照组比较,研究组血清AAT、VEGF、IL-1β水平增高,MMEF、FEV1、FVC、FEV1/FVC降低,差异有统计学意义(P<0.05)。COPD患者血清AAT、VEGF、IL-1β水平与MMEF、FEV1、FVC、FEV1/FVC呈显著负相关关系(P<0.05)。结论 COPD患者血清AAT、VEGF、IL-1β水平异常增高,这些指标与肺功能存在显著负相关关系,临床可通过测定上述指标对COPD患者肺功能进行综合评估。
Abstract:Objective To explore the correlation and value of α1 antitrypsin(AAT), vascular endothelial growth factor(VEGF), interleukin-1β(IL-1β) with pulmonary function in chronic obstructive pulmonary disease(COPD). Methods The clinical data of 178 patients with COPD in department of respiratory medicine of the hospital from November 2019 to November 2022 were retrospectively selected as study group, and the clinical data of 178 patients with physical examination at the same time were selected as control group. 5 mL of fasting venous blood was drawn from all subjects in the morning, and the levels of AAT, VEGF and IL-1β in serum were measured by immunoturbidimetry and ELISA. Pulmonary function [maximum mid expiratory flow(MMEF), forced expiratory volume in the first second(FEV1), forced vital capacity(FVC), FEV1/FVC] were measured. The levels of serum AAT, VEGF, IL-1β and pulmonary function indexes in the two groups were analyzed, and the correlation between AAT, VEGF, IL-1β and lung function indexes was analyzed. Results Compared with control group, the serum levels of AAT, VEGF and IL-1β in study group were increased, and the difference was statistically significant(P<0.05). Compared with control group, MMEF, FEV1, FVC and FEV1/FVC in study group were decreased(P<0.05). Serum levels of AAT, VEGF, IL-1β were negatively correlated with MMEF, FEV1, FVC and FEV1/FVC in patients with COPD(P<0.05). Conclusion The levels of serum AAT, VEGF and IL-1β in patients with COPD are abnormally increased, and there is a significant negative correlation with lung function. The above indicators can be measured clinically for comprehensive evaluation of pulmonary function in patients with COPD.
