目的:探讨不同手术方式治疗先天性马蹄内翻足治疗效果。方法:回顾性分析202例(298足)先天性马蹄内翻足患者的临床资料,根据患者的年龄、内翻足畸形和僵硬程度的不同分别采用跟腱延长+软组织松解、跖筋膜松解、跟腱延长+骰骨截骨或胫前肌外移、跟腱延长+距骨切除、跟腱延长+三关节融合+距骨切除等手术方式进行治疗。结果:202例平均随访时间10个月,优136例,良30例,可2例,差34例,优良率达82.2%。结论:严格把握手术指征、准确定位、仔细操作,正确配合矫形石膏和支具的使用,可保证手术的成功,外观和功能较满意。

Objective: To explore the therapeutic efficacy of all categories of operation in congenital talipes equinovarus. Methods: From June 2002 to June 2007, 202 patients 298 feet with congenital talipes equinovarus admitted in our hospital, and the operations such as the Achilles tendon lengthening+solution of soft tissue, the solution of metatarsus fascia, the Achilles tendon lengthening+cuboid bone osteoectomy or tibialis anterior tendon transfer, the Achilles tendon lengthening+talus excision, the Achilles tendon lengthening+fusion of tri-joints+talus excision etc, were adoptted. Results: The 202 patients were all followed-up, 10 months on everage.To evaluate according to Huang’s standard, the result were: the excellent were136 patients, the good were 30 patients, the middle were 2 patients and the bad were 34 patients. The good-excellent rate was 82.2. Conclusions: Choosing the different operations according to the age and the difference of varus, adduction, plantar flexion, degree of stiffness; grasping the operative indication strictly; operating carefully; applying the reshaping-plaster and orthosis correctly, all of above make sure the success of the operation and get the satisfactory effect on appearance and function.
