血清PTX3、TARC与银屑病治疗效果的关系

摘要:

目的 明确银屑病患者血清正五聚蛋白3(PTX3)、胸腺活化调节趋化因子(TARC)表达特点,分析其与银屑病患者治疗效果之间的具体关系。方法 选取2020年1月至2023年12月期间于本院接受治疗的120例银屑病患者作为研究对象,根据治疗8周后疗效分为有效组(86例)和无效组(34例)。收集两组患者临床资料,分析治疗前血清PTX3、TARC与银屑病治疗效果的关系。结果 有效组和无效组银屑病患者的年龄、银屑病面积和严重程度指数(PASI)评分、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)、白细胞计数(WBC)、中性粒细胞计数、红细胞沉降率(ESR)、白细胞介素(IL-8)、PTX3、TARC水平以及病情程度差异有统计学意义(P均<0.05)。多因素Cox回归分析显示,PASI评分、PCT、TNF-α、IL-8、PTX3、TARC均为银屑病治疗无效的独立危险因素(P<0.05)。ROC曲线表明,PTX3(AUC=0.890)、TARC(AUC=0.860)对于银屑病患者治疗效果有较高的预测能力,且显著优于PCT(AUC=0.793)、PASI评分(AUC=0.836)、TNF-α(AUC=0.782)和IL-8(AUC=0.820),P<0.001。Kaplan-Meier曲线和Lowess曲线结果表明,随着PTX3、TARC表达水平增高,治疗有效率显著降低。危险度分析结果显示,银屑病PTX3、TARC高表达患者治疗后无效风险分别为低表达患者的1.122、1.325倍(P均<0.05)。结论 PTX3、TARC是银屑病治疗无效的独立危险因素,具有较好的预测价值,可作为潜在的临床早期诊断指标。

Abstract:

Objective To clarify the expression characteristics of serum n-pentamerprotein 3(PTX3) and thymus activation-regulated chemokine(TARC) in patients with psoriasis, and to analyze the specific relationship between them and the therapeutic effect of psoriasis patients. Methods A total of 120 patients with psoriasis who were treated in the hospital from January 2020 to December 2023 were selected as research subjects. According to the treatment effect after 8 weeks of the treatment, the patients were divided into the effective group(86 cases) and the ineffective group(34 cases). Collected the clinical data of the patients and analyzed the relationship between serum PTX3, TARC and the therapeutic effect of the patients with psoriasis. Results The age, psoriasis area and severity index(PASI) score, high-sensitivity C-reactive protein(hs-CRP), procalcitonin(PCT), tumor necrosis factor(TNF-α), white blood cell count(WBC), neutrophil count, erythrocyte sedimentation rate(ESR), interleukin(IL-8) and positive of the patients with psoriasis in the effective group and the ineffective group. There were statistically significant differences in the levels of pentoxin-3(PTX3), thymus activation-regulated chemokine(TARC) and the severity of the disease(all P<0.05). Multivariate Cox regression analysis showed that PASI score, PCT, TNF-α, IL-8, PTX3 and TARC were all independent risk factors for ineffective treatment in patients with psoriasis(P<0.05). The ROC curve indicates that PTX3(AUC=0.890) and TARC(AUC=0.860) had a relatively high predictive ability for the therapeutic effect of the patients with psoriasis. And it was significantly superior to PCT(AUC=0.793), PASI score(AUC=0.836), TNF-α(AUC=0.782) and IL-8(AUC=0.820). The results of the Kaplan-Meier curve and the Lowess curve indicated that with the increase of the expression levels of PTX3 and TARC, the effective rate of treatment were decreased significantly. The results of risk analysis showed that the risk of ineffective treatment for the patients with high expression of PTX3 and TARC in psoriasis was 1.122 and 1.325 times that of the patients with low expression, respectively(all P<0.05). Conclusion PTX3 and TARC are independent risk factors for ineffective treatment in patients with psoriasis, with good predictive value and can be used as potential early clinical diagnostic indicators.
