肌电生物反馈联合下肢关节核心肌群训练在TKA术后患者的康复效果

摘要:

目的 研究肌电生物反馈联合下肢关节核心肌群训练在全膝关节置换术(Total knee arthroplasty, TKA)术后患者的康复效果。方法 回顾性分析海军青岛特勤疗养中心2021年5月-2023年11月收治的117例TKA术后患者的病历资料,随机分为观察组(n=59)和对照组(n=58),对照组采用肌电生物反馈治疗仪进行肌电生物反馈治疗,观察组采用肌电生物反馈联合下肢关节核心肌群训练治疗。干预12周后对比2组患者膝关节功能、膝关节疼痛程度、等速肌力参数、下肢运动功能、Barthel指数(BI)评分、膝关节协会评分(KSS)及并发症发生情况。结果 与本组干预前比较,干预后患者Lysholm膝关节评分、疲劳指数(WF)、总做功量(TW)、峰力矩(PT)、改良步态效能评分(mGES)、标准化步幅、BI评分与KSS评分升高,视觉模拟评分法(VAS)评分、坐站转换时间(STS)降低,差异有统计学意义(P<0.05)。与对照组干预后比较,观察组干预后Lysholm膝关节评分、WF、TW、PT、mGES评分、标准化步幅、BI评分和KSS评分均升高,VAS评分和STS均降低,差异有统计学意义(P<0.05)。2组并发症总发生率比较差异无统计学意义(P>0.05)。结论 肌电生物反馈联合下肢关节核心肌群训练可促进TKA患者术后关节功能康复,减轻膝关节疼痛程度,改善下肢运动功能与日常生活能力,增强抗疲劳能力。

Abstract:

Objective To study the effect of myoelectric biofeedback combined with lower limb joint core muscle training on rehabilitation of the patients after total knee arthroplasty(TKA). Methods The medical records of 117 TKA patients admitted to Qingdao Navy Special Service Nursing Center from May 2021 to November 2023 were retrospectively analyzed, and the patients were divided into observation group(n=59) and control group(n=58) according to different intervention methods. The control group received myoelectric biofeedback, and the observation group received joint core muscle training on the basis of the control group. Knee function, knee pain degree, isokinetic muscle strength parameters, lower limb motor function, Barthel index(BI) score, knee association score(KSS) and complications were compared between the 2 groups. Results Compared with the group before intervention, Lysholm knee joint score, work fatigue(WF), total work done(TW), peak moment(PT), modified gait effectiveness score(mGES), standardized stride length, BI score and KSS score were increased after intervention. Visual analog scale(VAS) score and sting-to-station conversion time(STS) were decreased, and the difference was statistically significant(P<0.05). Compared with control group after the intervention, Lysholm knee joint score, WF, TW, PT, mGES score, standardized stride length, BI score and KSS score were increased in the observation group after intervention, while VAS score and STS were decreased, with statistical significance(P<0.05). There was no difference in the total incidence of complications between the 2 groups(P>0.05). Conclusion The combination of myoelectric biofeedback and lower limb joint core muscle training can promote joint function rehabilitation, reduce knee pain, improve lower limb motor function and daily living ability and enhance anti-fatigue ability in TKA patients.
