本文报告了经胸骨正中切口,治疗39例重症二尖瓣狭窄伴房颤病人的经验。女性19例,男性20例,年龄20～51岁。心功能Ⅲ级者24例(62％),Ⅳ级者15例(38％)。闭式扩张术33例,直视扩张4例,瓣膜置换术2例。全组死亡3例。文章着重讨论了手术方法、病例选择、各种并发症的预防和治疗。
Abstract：The authors report the clinical experience with the use of mid-sterntomy for severe mitral stenosis, From April 1983 through May 1987, 39 cases were performed in the author's hospital. Among these patients, there were 20 males and 19 females, age ranged from 20-51 years. The heart functions of the patients belong to Grade Ⅲ in 24 (62％) and Grade Ⅳ in 15 (38％). 37 cases had atrial fibrillation and had history of heart failure. The chest X-ray examination revealed large hearts, with cardiothoracic ratio more than 60％ in all cases, Close mitral commissurotomy was done in 33. The mitral valve replacement in 2 and the open mitral eommissurotomy in 4. Three patients died early. The valve orifice was found to be about 0.7-1.2cm during operation in all cases, and was dilated to 3.0-3.5cm. Stress was put on selection of suitable cases, adequate preoperative preparation, and management of Low-Cardiac Output syndrome etc. Short term follow-up appeared to be satisfactory. Their diagnosis and the surgical technique were also discussed.
