目的 分析慢性疲劳综合征(Chronic fatigue syndrome, CFS)患者的中医证型分布规律并比较不同中医证型CFS患者的免疫功能情况。方法 选取2022年1月至2023年5月于上海中医药大学附属岳阳中西医结合医院确诊为CFS的患者共194例，对不同中医证型CFS患者的主要症状积分、免疫功能情况进行比较，分析其与一般资料的关系，并采用二元Logistic回归分析其影响因素。结果 194例CFS患者中脾肾阳虚患者82例(42.27%),占比最高，不同中医证型CFS患者的年龄、病程、高血压、糖尿病、既往史比较差异有统计学意义(P<0.05);不同中医证型CFS患者的T细胞、NK细胞、CD4~+T、CD8~+T、CD4/CD8水平比较差异有统计学意义(P<0.05),其中脾肾阳虚证型的患者T细胞、NK细胞、CD4~+T、CD4/CD8水平较气血两虚、肝郁脾虚及其他中医证型明显降低(P<0.05),而CD8~+T水平较气血两虚、肝郁脾虚及其他证型明显升高(P<0.05);年龄、T细胞与脾肾阳虚型CFS相关(P<0.001),脾肾阳虚型CFS患者的年龄和T细胞增加风险分别是非脾肾阳虚型CFS的1.20倍和0.63倍。结论 脾肾阳虚是CFS的主要中医证型之一，不同中医证型CFS患者的免疫功能存在差异，其中脾肾阳虚证型的患者的免疫能力较其他证型较为明显，而且疲劳症状和免疫能力又存在一定的相关性。

Objective To analyze the distribution pattern of traditional Chinese medicine(TCM) syndrome types in patients with chronic fatigue syndrome(CFS) and compare the immune function of the patients with different syndromes of CFS. Methods A total of 194 patients diagnosed with CFS at the hospital from January 2022 to May 2023 were selected. The main symptom scores and immune function of CFS patients with different syndrome types were compared, and their relationship with general data was analyzed. Binary Logistic regression analysis of influencing factors. Results Among 194 CFS patients, 82(42.27%) had spleen kidney yang deficiency, accounting for the highest proportion.The age, course of disease, hypertension, diabetes and history of CFS patients with different syndromes were statistically significant(P<0.05). There was a statistically significant difference in the levels of T cells, NK cells, CD4~+T, CD8~+T and CD4/CD8 among patients with different syndrome types of CFS(P<0.05). Among them, the levels of T cells, NK cells, CD4~+T and CD4/CD8 in patients with spleen and kidney yang deficiency syndrome were significantly lower than those of qi and blood deficiency, liver depression and spleen deficiency and other syndrome types(P<0.05), while the levels of CD8~+T were significantly higher than those of qi and blood deficiency, liver depression and spleen deficiency and other syndrome types(P<0.05). Age, T cells and spleen and kidney yang deficiency type CFS were correlated(P<0.001);The age and T-cell increase risk of patients with spleen and kidney yang deficiency type CFS were 1.20 and 0.63 times higher than those without spleen and kidney yang deficiency type CFS, respectively. Conclusion Spleen and kidney yang deficiency is the main traditional Chinese medicine(TCM) syndrome type of CFS, and there are differences in immune function among different syndrome types of CFS patients. Among them, the immune ability of patients with spleen and kidney yang deficiency syndrome is more obvious than other syndrome types, and there is a certain correlation between fatigue symptoms and immune ability.
