目的:探讨爱泼斯坦 -巴尔病毒 (EBV)与鼻咽癌发生、发展的关系及其临床病理学意义。方法 :应用免疫组化技术检测 4 6例鼻咽癌、2 8例鼻咽癌癌旁组织、36例慢性鼻咽炎石蜡切片组织中爱泼斯坦 -巴尔病毒受体(EBER- 1)的表达。结果:鼻咽癌组织中 EBER- 1表达率为 74 .0 % ,且在转移灶中不丢失 ,癌旁组织、慢性鼻咽炎组织中 EBER- 1表达率分别为 32 .1%、16 .7%。结论:检测 EBER- 1的表达对鼻咽癌诊断与鉴别诊断具有重要价值。慢性鼻咽炎组织、癌旁组织中 EBER- 1阳性者提示为高危癌前病变 ,应密切随访监测 ,可能有利于早期发现鼻咽癌

Objective: To study the relationship between Epstein Bar virus(EBV) and the development of nasopharyngeal carcinoma(NPC) as well as the clinicopathological significance of EBV expression. Methods: The expression and distribution of EBER 1 in paraffin embedded tisssue from 46 cases of NPC were studied by immunohistochemistry. Results: EBER 1 was not detected in normal and simple hyperplasia of nasopharyngeal epithelial cells, metaplasia of nasopharyngeal mucosal epithelium and other tumors of head and neck. Of the 36 cases of NPC, 44 cases (74%) showed the expression of EBER 1 that was still present in metastatic Npc cells and dysplastic cells of paratumourous nasopharyngeal epithelium. Conclusions: There is a strong association between EBV and NPC. EBV may play an important role in the pathogenesis of NPC, and EBER 1 expression is useful in the diagnosis and differential diagnosis of NPC. The dysplastic cells of nasopharyngeal epithelium especially those positive for EBER 1 can be regarded as a kind of precarcinomatous change. The measurement of EBER 1 may be helpful in screening early NPC in high risk populations.
